PAYMENT POLICY

Medical services are provided and billed by Medical Clinic. Our providers participate in most all of the major health insurance plans in the region. Following are the health plans which we currently accept.

If your plan is not listed, we will be unable to file a claim on your behalf. However, we can provide you with a form that you can submit to your insurance company to instruct them to send reimbursements directly to you.

Please be aware that with any health plans, there may be deductibles and copayments for which you could be responsible.

QUESTIONS ABOUT YOUR BILL

Billing

There will be three (3) bills or insurance claims generated for your surgery.

  1. Facility Fee from the Surgery Center

  2. Professional Fee from your Surgeon

  3. Anesthesia Fee from the Anesthesia Providers

 

If you have any questions regarding your bill or insurance claims please call the following numbers:

  1. Call the Eligibility Department for eligibility and copay information, 315-452-6911, x247 or x285.

  2. Call the Billing Department at 315-414-2238 or 315-451-6911 x 245 for the Surgery Center bill.

  3. Call your doctor’s office for the Surgeon’s professional bill.

  4. Call the billing office for Syracuse Anesthesia & Pain Management at 315-422-6705 for your anesthesia bill.

 

If you have no insurance then payment for services rendered will be due the day of the surgery.

 

If you feel you have a financial hardship, contact your doctor for financial alternatives.

Please bring your INSURANCE CARD AND PHOTO ID the day of surgery.

Note: Your co-pay is due the day of your surgery.
